Relocation Service Coordination (RSC)
Empowering Transitions to Community Living
RSC-TCM is a specialized service aimed at supporting eligible members residing in qualified institutions who aspire to transition into the community. Our goal is to facilitate a smooth, well-planned transition by connecting individuals with essential services like medical, social, educational, housing and resources tailored to their unique needs and circumstances.
Key Activities:
Comprehensive Service Planning:
- Development and implementation of personalized service plans.
- Referrals and coordination with service providers.
Client Advocacy:
- Establishing client and collateral contacts for effective plan execution.
- Ongoing monitoring and advocacy to ensure the well-being of our clients.
Community Integration:
- Assistance with identifying and securing suitable housing options.
- Support with housing application processes, even for those with a challenging rental history.
- Coordination of tours to prospective relocation sites.
Financial Assistance:
- Assisting individuals in applying for various income sources, including general assistance and social security benefits.
- Facilitating the eligibility process of waiver services through the State Medical Review Team (SMRT) or Social Security Administration (SSA).
Additional Support:
- Contacting relevant county agencies and initiating the relocation process.
- Assistance with record expungement advocacy.
- Coordination of essential services such as movers, household items, and furniture.
- Arrangement of transportation services.
Our Approach

We collaborate closely with individuals to develop comprehensive transition plans, ensuring a safe and successful move into the community. Our team is dedicated to providing ongoing support and guidance throughout the relocation process, fostering independence and empowerment.
Eligible Institutions:
- Hospitals
- Intermediate care facilities for persons with developmental disabilities or related conditions (ICF/DDs)
- Institutions for mental disease (IMDs); including regional treatment centers (RTCs) licensed as hospitals or nursing facilities
- Nursing facilities (NFs); including skilled nursing and certified boarding care facilities
- Facilities with additional federal designations such as IMDs or RTCs, provided they are licensed as hospitals, nursing facilities, or ICF/DDs.
Eligibility Criteria:
A member is eligible for RSC-TCM services when the following criteria are met:
- Eligible for MA (including Program IM, NM, or RM)
- Resides in an institution qualifying for services at the time of service delivery
- Chooses to transition into the community
- Chooses to receive RSC-TCM services
- TCM benefit is not exhausted
